How to apply

- Obtain the application form from the concerned medical institution.

- The nodal officer will review and forward the application to the Head of Department (HOD).
- The HOD will verify and send the application to the Screening Committee for approval.
- Attend the Screening Committee meeting, which occurs every alternate day, for application review.
- If approved, treatment will be provided at no cost up to the sanctioned amount.
